Why Are You Applying for Merchant Payments?
- Never Accepted Cards Before: If this is your first time accepting credit card payments, let us know. We’ll make sure to support you through this transition.
- Switching Payment Processors: If you’ve accepted card payments in the past but are looking to switch to Stax, this is where you can let us know.
Have You Ever Experienced an Account Data Compromise?
This question asks if your business has ever had a security breach where customer payment information was accessed without authorization. This helps us evaluate risks and ensure proper security measures are in place.
What Is Your Refund Policy?
Provide information about your business’s refund policy. Options include:
- No Refunds
- Merchant Exchange Only
- 30 Days or Fewer
- Other
How Do You Primarily Accept Payments?
Tell us how you expect to receive payments most often. Options include:
- In-Person (using a terminal or card reader)
- Online/E-commerce (through a website)
- Mobile (using a mobile reader or app)
- Mail or Telephone Order (MOTO)
What Products or Services Do You Sell?
Describe the type of products or services your business offers. This helps us understand your business model better and ensure compliance.
Processing Volumes
Annual Gross Credit Card Revenue
Estimate the total amount of sales you expect to process via credit cards annually. If you’re not sure, consider estimating your monthly sales and multiplying by 12.
Average Credit Card Transaction Size
Provide an estimate of your typical transaction amount. For example, if you run a coffee shop, this might be the average cost of a coffee and a snack.
Largest Credit Card Transaction Size
Estimate the highest transaction amount you might process. For example, for a boutique, this could be the price of a high-end item or a bulk purchase.
Transaction Breakdown
Provide an approximate breakdown of how you expect transactions to be conducted:
- Swiped/Dipped (Card Present): Percentage of transactions where the card is physically present.
- Keyed-In (Card Not Present): Percentage of transactions entered manually, such as through a phone order.
- International Transactions: Percentage of transactions involving cards issued outside the country.
ACH Payments (If Applicable)
If your business also accepts ACH payments (direct bank transfers), please provide estimates for:
- Annual gross ACH revenue: Estimate your total annual sales volume processed through ACH. If unsure, estimate your monthly ACH sales and multiply by 12.
- Average ACH transaction size: Provide an estimate of your typical ACH transaction amount. This might be higher or lower than your average credit card transaction.
- Largest ACH transaction size: Estimate the highest ACH transaction amount you might process.
